Comments:
An unannounced monitoring inspection was conducted Thursday, October 24, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 9:15 AM and concluded at approximately 12:45 PM. A total of 99 children were present during the inspection and the following staff to child ratios were observed:
Toddlers: 8 children and 2 staff
Twos: 9 children and 2 staff
Twos: 9 children and 2 staff
Threes: 11 children and 2 staff
Threes: 11 children and 2 staff
Threes: 11 children and 2 staff
Fours: 12 children and 2 staff
Fours: 13 children and 2 staff
JK: 11 children and 2 staff
Children were observed having their diapers changed, looking at books, riding toy cars, playing with a doctor kit, creating with cotton balls and band-aids while learning about community helpers, creating quilts, using lace cards, playing with toys, playing outside, washing hands and taking bathroom breaks and eating snack. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Ten children?s records and 10 staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of 10 staff records, the center did not ensure that all staff and volunteers had a search of the central registry within the first 30 days of employment or volunteering.
Evidence:
1. The record for Staff #8 (hire date 09/01/18) did not contain a central registry result.
2. A member of management confirmed this information during the inspection.

Plan of Correction: I will have Staff #8 complete the central registry request and send it to the agency. I will email a copy of the request to the licensing inspector within 10 days.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of 10 staff records, one record did not contain the required updated background checks.
Evidence:
The record for Staff #6 (hire date: 09/10/12) contained a sworn statement dated 09/07/12. Effective July 1, 2017, staff at all religiously exempt child day centers must repeat background checks every five years.

Plan of Correction: Corrected during the inspection.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on the review of the 10 records, the center did not request a search of out-of-state founded complaints of child abuse and neglect within the first 30 days of beginning employment for one staff member who has lived outside of Virginia within the last five years.
Evidence:
The record for Staff #3 (hire date 9/11/18) indicated the staff member has lived outside of Virginia within the last five years. The record did not contain documentation of a request for founded complaints of child abuse from the state the staff member indicated she had lived in within the last five years. A member of management stated an out-of-state child abuse search for Staff #3 has not been requested as of 10/24/19.

Plan of Correction: We will complete a child abuse and neglect search from the state Staff #3 lived in. A search will be requested within the next 10 days and a copy will be sent to the licensing inspector.
