Comments:
An unannounced monitoring inspection was conducted on October 25, 2019 from approximately 9:52 am-11:30 am. Upon arrival to the facility, there were 38 preschool students with six staff members. The children were observed participating in small group activities with their teachers and playing in Centers. Five classrooms, the Chapel, the Fellowship Hall and the outdoor play area were inspected. The center does not provide transportation.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, in one of 10 staff records reviewed and staff interview, the center did not obtain, within 30 days of employment, a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years.

Evidence: The Director confirmed that there was no documentation of an out of state Central Registry finding for staff #5 who indicated on the Sworn Statement dated 5/9/19, as having lived outside of the state of Virginia within the last five years.

Plan of Correction: The Director was unaware that an out of state check was needed because the prospective employee was not a regular staff member but was submitting paperwork for the possibility of being a substitute. An out of state request was submitted on October 16, 2019 and the results are pending and will be placed in the employee's file when they arrive. All other required background checks were done in a timely manner.
