The licensing inspector completed an inspection in response to a self-reported concern regarding supervision. The Department was notified of the incident on 09/16/19. On 09/16/19, the licensing inspector was on site from approximately 10:00AM until 2:00PM interviewing staff, conducting observations at the center, and reviewing documentation. Additional interviews were conducted on 10/01/19 and 10/23/19.
