Comments:

An unannounced monitoring inspection was conducted Friday, November 1, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 11:15 AM and concluded at approximately 1:45 PM. A total of 86 children were present during the inspection and the following staff to child ratios were observed: Twos: 9 children and 2 staff Twos: 8 children and 2 staff Twos: 8 children and 3 staff Threes: 13 children and 2 staff Threes: 11 children and 2 staff Threes: 7 children and 2 staff Fours: 16 children and 2 staff Fives: 14 children and 2 staff Children were observed having books read to them, using their writing journal, drawing at the easel, creating bracelets, building with blocks, playing with toy animals, playing outside, riding bikes, playing with toys, making pumpkins, building with blocks, washing hands and taking bathroom breaks and eating snack. Ten children?s records and 10 staff records were reviewed. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Transportation is not provided. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. One violation was cited during the inspection. Description:	Based on the review of the 10 staff records, the center did not request a search of out-of-state founded complaints of child abuse and neglect within the first 30 days of beginning employment one staff member who has lived outside of Virginia within the last five years. Evidence: The record for Staff #5 (hire date 9/1/16) indicated the staff member has lived outside of Virginia within the last five years. The records did not contain documentation of a request for founded complaints of child abuse from the state the staff member indicated she had lived in within the last five years. A member of management stated an out-of-state child abuse and neglect search had not been requested as of 11/01/19.
Plan of Correction:	It was an oversight that the staff member had lived in another state within the last five years. A form requesting the search was completed while the licensing inspector was on site.
