As of July 1, 2017, Background checks are required to be repeated every 5 years.

Comments:
An unannounced monitoring inspection was conducted beginning at 10:30 a.m. and ending at 1:30 p.m. with the director. There were 108 children in care ranging in age from 2 to 5 years of age under the care and supervision of 14 staff. Children were observed during circle time, practicing songs for a fall performance, restroom break and story time about Noah's Ark. Center documents and 22 staff records were reviewed.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of documentation and interview with staff, it was determined that the center did not have documentation of required sworn statement or affirmation by the first day of employment and central registry background checks by the 30th day of employment. Evidence: Staff #6, hired in 2015, did not have documentation of a sworn statement or central registry check results in the record. Staff #15, hired 2017, did not have documentation of central registry background check results in the record.

Plan of Correction: Background checks were conducted but documentation may have been misplaced. Once the documentation is located, it will be provided and filed in the staff record. If it cannot be located, new central registry background checks will be requested.
Staff #6 completed a sworn statement during the inspection.

Standard #: 63.2(17)-1716-A-6
Description: Based on review of documentation including the parent handbook and preschool website, it was determined that the center did not describe all aspects of the child day center's operations to parents and guardians of the children in the center and made available to the general public. Evidence: There was no statement regarding whether o not the center has public liability insurance.

Plan of Correction: This information will be added to the parent handbook as soon as possible.

Description: Based on review of documentation and interview with staff, it was determined that the center did not ensure that sworn statement or affirmations and central registry background checks were conducted every five years as required.
Evidence:Staff #2's most recent sworn statement was dated 7/19/2011. Staff #3's most recent sworn statement was dated 8/29/2012 and central registry background check was dated 8/13/2008. Staff #4's most recent sworn statement was dated 8/29/2012. Staff #5's most recent sworn statemnt was dated 7/14/2008. Staff #7's most recent sworn statement was dated 8/29/2012. Staff #10's most recent sworn statment was dated 8/29/2012 and central registry background check was dated 8/13/2008. Staff #11's most recent sworn statement was dated 8/29/2012. Staff #16's most recent sworn statment was dated 8/29/12. Staff 18's most recent sworn statment was dated 8/29/2012. Staff 20's most recent sworn statement was dated 7/14/2014 and central registry background check was dated 8/21/2014. Staff 22's most recent sworn statement was dated 6/6/2014 and central registry background check was dated 8/21/2014.

Plan of Correction: Staff completed updated sworn statement forms during the inspection. Staff with missing central registry background checks completed the forms during the inspection and they were notarized and prepared to be mailed.
