63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on 11/4/19 from 9:35 am until 11:15 am at Woodstock Presbyterian Church to review the religious exempt requirements. At the time of the inspection 17 children were present with two staff. The sample size consisted of five children's records and seven staff's records. Children and staff were observed during indoor free play, structured activities, hand washing, snack, transitions and behavioral management. Violations were found during this inspection and are documented on the violation notice.

Violations:
Standard #: 63.2(17)-1716-A-4
Description: Based on record review and interview, the center failed to ensure all staff have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him/her from caring for children under his/her supervision prior to start date and updated every year.

Evidence:

1. The records of seven staff were reviewed. Four did not contain staff health reports.
2. The director stated they do not have substitutes complete a staff health report and all the ones missing a staff health report are substitute teachers.

Plan of Correction: Any staff that is paid, used in ratio and/or left alone with children will be required to provide a staff health report prior to working with children and yearly there after.

Description: Based on a review of the staff handbook and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards.

Evidence:

1. A container of Clorox wipes was on a floor shelf beside a table used by children.
2. The director stated hazardous substances are to be kept out of reach of children, however the Clorox wipes are used at that station on a regular basis.

Plan of Correction: The Clorox wipes were immediately moved out of reach of children.

Description: Based on record review and interview, the center failed to establish and implement a procedure to ensure that all staff are able to recognize the signs of child abuse and neglect.

Evidence:

1. The records of seven staff were reviewed. Four staff's records did not contain documentation of completing training in recognizing signs of child abuse and neglect.
2. The director stated all four staff are substitutes and the center has never required substitutes to obtain training in recognizing signs of child abuse and neglect.

Plan of Correction: Any staff that is paid, used in ratio and/or left alone with children will be required to complete training in recognizing signs of child abuse and neglect.
