Comments:

The Licensing Inspector for Trinity United Methodist Church day care conducted an unannounced RECDC Monitor Inspection on Wednesday, November 6, 2019. Licensing Inspector arrived & started inspection at 12:10 PM and concluded at 2:25 PM. There were thirty-six (36) children ages 5 months-4 years old under the direct care and one (1) substitute staff person & ten (10) staff members present during today?s inspection. Licensing Inspector reviewed information for eleven (11) staff members. Areas of non-compliance and found during today?s inspection along with the risk assessment were reviewed with the Center Director on 11/06/2019. Standard #:	63.2(17)-1716-A
Description:	Based on inspection, the Center failed to disclose in writing to the parents or guardians of the children in the center the fact that it is exempt from licensure and has posted the Exemption Letter in a visible location on the premises and the qualifications of the personnel employed therein. EVIDENCE: Required written disclosures have not been provided to the parents or guardians of the children in the center. Required letter of exemption was not posted in a visible location on the premises.
Plan of Correction:	Center Director will post the letter of exemption. The staff qualifications will be included in the parent handbook. The Center Director will complete and post the written disclosures.
Standard #:	63.2(17)-1716-A-4
Description:	Based on review of staff information, the Center failed to ensure each employee has been certified by a practicing physician to be free from any disability which would prevent him from caring for children under his supervision. EVIDENCE: Licensing Inspector reviewed health information for eleven (11) staff members. Evidence of Staff Health Report was unavailable for Staff #4.
Plan of Correction:	Staff will complete health exam by a physician. Upon completion, documentation of the findings will be maintained on file.

Description:	' Based on review of center information, the Center failed to provide a written statement regarding physical facilities, enrollment capacity, food services, health requirements for the staff and public liability insurance to the parents/guardians of the children in the center and made available to the general public. EVIDENCE: A written description of the center's physical facilities, information regarding the center's health requirements for staff and information regarding the center's public liability insurance was not made available to the parents or the general public.
Plan of Correction:	Center Director will include required written disclosures in the parent handbook. Center will distribute written copy of the handbook to all parents upon completion. The Center Director will complete and post the written disclosures.
