Comments:
An unannounced monitoring inspection was conducted beginning at 12:15 p.m. and ending at 2:00 p.m. with the head of school and office manager. There were eight children in care ages three and four years old under the care and supervision of 2 staff. Children were observed during lunch. Lunch is brought from home. Center documents and two staff records were reviewed.

Standard #: 63.2(17)-1716-A
Description: Based on review of documentation and interview with staff, it was determined that the center did not provide required written disclosures to the parents and guardians of the children in the center. Evidence: There was no written disclosure regarding qualifications of personnel.

Plan of Correction: This information will be updated as soon as possible.

Standard #: 63.2(17)-1716-A-6
Description: Based on review of documentation and interview with staff, it was determined that the center did not describe the required aspects of the center's operations in writing. Evidence: The following aspects of the child day center's operations were not described in a written statement that was provided to the parents or guardians of the children in the center and made available to the general public: description of the physical facilities, health requirements for the staff and public liability insurance.

Plan of Correction: The public disclosure statement will be completed and posted on the premises.

Standard #: 63.2(17)-1716-B-3
Description: Based on interview with staff, it was determined that the center did not ensure that a daily simple health screening and exclusion of sick children was performed by a person trained to conducts health screenings. Evidence: There was no documentation of staff training regarding daily simple health screenings or Daily Health Observation training

Plan of Correction: Staff will complete training.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of documentation, it was determined that the center did not obtain central registry background checks every five years as required. Evidence: Staff #1, hired 2003, most recent central registry background check was dated 10/23/2014.

Plan of Correction: A new central registry background check will be conducted as soon as possible.
