An unannounced monitoring inspection was conducted beginning at 1:15 p.m. and ending at 3:30 p.m. with the director. There were 28 children in care ranging in age from 4 to 5 years of age under the care and supervision of 4 staff. Children were observed during nap and library. Center documents and 16 staff records were reviewed.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of documentation, it was determined that the center did not obtain sworn statement or affirmation by the first day of employment and central registry background check by the 30th day of employment. Evidence: Staff #3, hire date 8/2019 and Staff #5, hire date 8/2019, did not have documentation of sworn statement or central registry background check in the record. Staff #6, hire date 9/2018, and Staff #9, hire date 9/2018, did not have documentation of central registry in the record. Plan of Correction: Background checks were being conducted by the Diocese of Arlington and documentation was not obtained for the file. Background checks will be conducted as soon as possible.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of documentation, it was determined that the center did not obtain national fingerprinting background checks prior to the first day of employment. Evidence: Staff #5, hired 8/2019, and Staff #12, hired 6/26/2015, did not have documentation of fingerprint background checks in the record. Plan of Correction: Background checks were being conducted by the Diocese of Arlington and documentation was not obtained for the file. Background checks will be conducted as soon as possible.
