An unannounced Code compliance inspection was conducted today, 11/6/19. Upon arrival to the center the children were taking a nap. There are 15 children in care with two staff in the classroom and one staff conducting parent/teacher conferences. A review of staff and children's records was also conducted during today's inspection.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of staff records the center failed to have a staff complete a sworn disclosure statement and a central registry check within the set time frames.
Evidence:
1. Staff #1, 3 & 4 did not have a sworn disclosure statement on file prior to the first day of employment.
2. Staff #2 & 6 have a hire date of 8/21/17 and there are no central registry record check on file.Plan of Correction: The director with the assistance of the principal will ensure these forms are completed.
Standard #: 63.2(17)-1716-A Description: Based on review of written disclosure information for the parents the center failed to include all the required information.
Evidence:
1. The following information was not provided to parents in writing: qualifications of personnel, physical facilities, health requirements for staff and public liability insurance coverage.Plan of Correction: The director will ensure that this information is provided to the parents/guardians in writing.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff files the center failed to have all staff submit to a fingerprint-based criminal history check prior to the first day of employment.
Evidence:
1. Staff # 2 (hire date 8/21/17/), #3 (hire date 6/25/13), #4 (hire date 1/1/10) and #6 (hire date 8/21/17 do not have a fingerprint based criminal history check on file.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records the center failed to ensure that staff residing outside of the state of Virginia in the past five years have an out-of-state child abuse and neglect search on file.
Evidence:
1. Staff #5 has a hire date of 4/29/19 and there is no out of state background check conducted by the end of the 30th day of employment.Plan of Correction: Not available online. Contact Inspector for more information.
