Comments:
An unannounced monitoring inspection was conducted Tuesday, October 29, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 1:20 PM and concluded at approximately 3:00 PM. A total of 8 children being supervised by two staff were present during the inspection.
Children were observed napping, looking at books, taking bathroom breaks and washing hands during the inspection. Five children?s records and two staff records were reviewed. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Transportation is not provided. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Standard #: 22VAC40-191-40-D-2
Description: Based on review of three staff records, the center did not ensure that all staff and volunteers had a sworn statement or affirmation prior to the beginning of employment and a search of the central registry within the first 30 days of employment or volunteering.
Evidence:
The records for Staff #1 (worked with the center for over 20 plus years) and Staff #2 (worked with the center for over 14 years) did not contain central registry results and completed sworn statements.

Plan of Correction: I have the Sworn Statement and Central Registry for both employees.

Standard #: 63.2(17)-1716-A-5-a
Description: Based on record review, interview and observation, the religiously exempt child day center did not ensure compliance with the terms of the religious exemption issued by the Virginia Department of Social Services.
Evidence:
1. The letter issued by the Virginia Department of Social Services stated ?Salem Christian School, located at 1700 Cardwell Road, Crozier, Virginia, 23039 has filed the required documentation for an exemption as a child day center that is operated or conducted under the auspices of a religious institution pursuant to Section 63.2-1716 of the Code of Virginia?.
2. On 10/29/19, an attempt was made to conduct a Religious Exempt Child Day Center inspection at the center located 1700 Cardwell Road, Crozier, Virginia, 23039. The building located at this address was vacant.
3. An internet search was conducted and information indicated Salem Christian had relocated. An inspection was conducted at 35 Church Lane, Gum Spring, VA.
4. A staff member stated the center relocated in July 2019.

Plan of Correction: The notifying and applying for a current DSS exemption for this location is under way. I have the health inspection finished and the fire inspection is done but not complete yet. We are having to fix a couple of things. Also, the staff ratios are almost complete and the statement of intent is done.

Description: Based on review of documentation, the center did not describe in written statement to parents or guardians and made available to general public aspects regarding the center's operations concerning physical facilities and enrollment capacity.
Evidence:
The information reviewed in writing that is distributed to parents or guardians and made available to general public did not contain information describing physical facilities and enrollment capacity.
