Comments:
An unannounced monitoring inspection was conducted Tuesday, November 4, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 1:00 PM and concluded at approximately 4:00 PM. A total of 97 children were present during the inspection and being supervised by eight staff. The required staff-to-child ratios were being maintained during the inspection.
Children were observed taking bathroom breaks and washing their hands, eating snack, completing homework and playing outside. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Ten children?s records and eight staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Standard #: 22VAC40-191-40-D-2
Description: Based on review of eight staff records, the center did not ensure that all staff and volunteers had a sworn statement or affirmation prior to the beginning of employment.
Evidence:
1. The records for Staff #2 (hire date 08/16), Staff #3 (hire date 08/16), Staff #4 (hire date 08/17) and Staff #7 (hire date 08/20/19) did not contain a completed sworn statement.

Plan of Correction: The Sworn Statements were completed and added to the staff records.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of eight staff records, the center did not obtain the required updated background checks for four staff members.
Evidence:
1. The record for Staff #1 (hire date: 08/09) contained a sworn statement dated 08/27/2012.
2. The record for Staff #5 (hire date: 08/09) contained a sworn statement dated 10/16/2012.
3. The record for Staff #6 (hire date: 01/11) contained a sworn statement dated 08/16/2012.
4. The record for Staff #8 (hire date: 09/09) contained a sworn statement dated 08/23/2012.
5. Effective July 1, 2017, staff at all religiously exempt child day centers must repeat background checks every five years.

Plan of Correction: The Sworn Statements will be completed and added to the staff records. We will ensure all background checks are updated at least every five years.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of eight staff records, the center did not obtain satisfactory results of the finger-print based national criminal background check for one staff prior to employment, and did not obtain satisfactory results of the fingerprint-based national criminal background check by 09/30/2018 for seven staff hired prior to 1/22/2018.
Evidence:
1. The record for Staff #1 (hire date 08/09) contained satisfactory results of the finger-print based national criminal background check dated 04/17/2019.
2. The record for Staff #2 (hire date 08/16) contained satisfactory results of the finger-print based national criminal background check dated 04/18/2019.
3. The record for Staff #3 (hire date 08/16) contained satisfactory results of the finger-print based national criminal background check dated 05/07/2019.
4. The record for Staff #4 (hire date 08/17) contained satisfactory results of the finger-print based national criminal background check dated 04/26/2019.
5. The record for Staff #5 (hire date 08/09) contained satisfactory results of the finger-print based national criminal background check dated 04/18/2019.
6. The record for Staff #6 (hire date 01/11) contained satisfactory results of the finger-print based national criminal background check dated 04/24/2019.
7. The record for Staff #8 (hire date 09/09) contained satisfactory results of the finger-print based national criminal background check dated 04/17/2019. Staff hired prior to 01/22/2018 was required to have the results of the national criminal background check by 09/30/2018.
8. The record for Staff #7 (hire date 08/20/19) contained satisfactory results of the finger-print based national criminal background check dated 08/26/2019. The center is required to obtain satisfactory results of the finger-print based criminal record check prior to staff working for all staff hired on or after 01/22/2018.

Plan of Correction: We will require the results of the criminal background check prior to staff beginning employment.
