Comments:
An unannounced monitoring inspection was conducted Wednesday, November 6, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 9:00 AM and concluded at approximately 11:00 AM. A total of 18 children were present during the inspection and the following staff to child ratios were observed:
Threes: 7 children and 2 staff
Fours: 11 children and 1 staff
Children were observed participating in circle time, learning nursery rhymes with felt board pieces, writing letters, creating elephants from paper plates, coloring fire engines, reading books, participating in share time, taking a walk to look at the autumn leaves and creating Elmer the elephant. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Five children?s records and four staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Standard #: 22VAC40-191-40-D-2
Description: Based on review of four staff records, the center did not ensure that all staff and volunteers had a sworn statement or affirmation prior to the beginning of employment.
Evidence:
1. The records for Staff #1 (employed for over 21 years), Staff #2 (employed for over 2 years), Staff #3 (employed for over 12 years) and Staff #4 (employed for over 7 years) did not contain a completed sworn statement.
2. A member of management confirmed this information during the inspection.

Plan of Correction: A copy of the sworn statement was printed during the inspection and completed by the staff.

Standard #: 63.2(17)-1716-A-6
Description: Based on review of documentation and interview, the center did not describe in written statement to parents or guardians and made available to general public aspects regarding the center's operations concerning physical facilities, enrollment capacity and public liability insurance.

Evidence:
Documentation that is given to parents and guardians and made available was reviewed. The documentation reviewed did not contain information describing physical facilities, enrollment capacity, and public liability insurance. Staff #4 stated she and the preschool committee were working on including this information in the paperwork.

Plan of Correction: We are aware this information needs to be included and the church and myself are completing the disclosure information form.

Standard #: 63.2(17)-1716-B-3
Description: Based on a review of records and interview, the center did not ensure there was at least one staff member present that had been trained to conduct daily simple health screenings to exclude children as necessary.
Evidence:
Four staff records were reviewed and did not contain documentation of daily simple health screening. Staff #4 stated the staff look at the children when they arrive but they have not completed an official training to conduct daily simple health screenings.

Plan of Correction: The licensing inspector will send information on obtaining the health screening training and it will be completed by staff.
