Comments:
An unannounced monitoring inspection was conducted Tuesday, November 12, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 1:00 PM and concluded at approximately 3:00 PM. A total of 3 children were present during the inspection being supervised by two staff members.
Children were observed playing with toys, coloring, hanging trees that were made, playing with balls and being picked up by their families. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Five children?s records and four staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Standard #: 22VAC40-191-40-D-2
Description: Based on review of four staff records, the center did not ensure that all staff and volunteers had a sworn statement or affirmation prior to the beginning of employment and a search of the central registry within the first 30 days of employment or volunteering.
Evidence:
1. The records for Staff #1 (hire date 9/6/16), Staff #2 (hire date 8/19) and Staff #3 (hire date 3/27/17) did not contain central registry results and completed sworn statements.

Plan of Correction: We completed the forms for the required background checks during the inspection.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of four staff records, the center did not ensure one record contained the required updated background checks.
Evidence:
The record for Staff #4 (hire date: 06/2012) contained a central registry finding dated 08/17/2012 and a sworn statement dated 06/18/2012. Effective July 1, 2017, staff at all religiously exempt child day centers must repeat background checks every five years.

Plan of Correction: I was not aware the background checks were required to be updated every five years. The forms for the background checks were completed during the inspection.
