An unannounced monitoring inspection was conducted Tuesday, November 12, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 10:00 AM and concluded at approximately 12:00 PM. A total of 15 children were present during the inspection and the following staff to child ratios were observed:
Threes: 6 children and 2 staff
Fours: 9 children and 2 staff
Children were observed working puzzles, coloring, creating owls, playing in dramatic play center, learning about the letter ?Hh? and creating houses, taking bathroom breaks and washing hands and eating snack. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Five children?s records and six staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Standard #: 22VAC40-191-40-D-2 Description: Based on review of six staff records, the center did not ensure that all staff and volunteers had a sworn statement prior to beginning employment.
Evidence:
1. The records for Staff #1 (hire date 8/04), Staff #2 (hire date 08/16), Staff #3 (hire date 08/15), Staff #4 (hire date 1/11), Staff #5 (hire date 8/16) and Staff #6 (hire date 8/19) did not contain a completed sworn statement.
2. Staff #1 stated the sworn statements have been completed and could not be located during the inspection.Plan of Correction: I will send a copy of the sworn statements once they are located or I will have the staff complete the sworn statements again.
Standard #: 63.2(17)-1716-B-4 Description: Based on the review of five children's records, the center did not establish and implement procedures to ensure that three children in the center are in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases.
Evidence:
The records for Child #1 (first day of attendance 09/18), Child #3 (first day of attendance 09/18) and Child #4 (first day of attendance 09/17) did not have documentation of immunizations. Staff #1 stated a copy of the immunizations have been obtained but they could not be located during the inspection. The parent handbook states a copy of immunizations are required to be submitted prior to attending the center.Plan of Correction: A copy of the immunizations have been obtained but could not be located during the inspection. If the immunization records cannot be located, I will obtain another copy of the records from the families.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of five records and interview, the center did not obtain satisfactory results of the finger-print based national criminal background check for one staff prior to employment, and did not obtain satisfactory results of the fingerprint-based national criminal background check by 09/30/2018 for five staff hired prior to 1/22/2018.
Evidence:
The records for Staff #1 (hire date 8/04), Staff #2 (hire date 08/16), Staff #3 (hire date 08/15), Staff #4 (hire date 1/11) and Staff #5 (hire date 8/16) did not contain satisfactory results of the fingerprint-based national criminal background check by 09/30/2018.
The record for Staff #6 (hire date 8/19) did not contain satisfactory results of the fingerprint-based national criminal background check.Plan of Correction: All staff have completed the fingerprint-based national background checks. We have not received the results. We believed we would only receive results if the background checks were not satisfactory. We will reach out to the Office of Background Investigations to obtain the results.
