Comments:
A Subsidy Health and Safety inspection was conducted on 11/13/2019 to review supplemental health and safety requirements. Children were observed, and related children and staff files were reviewed along with the physical plant. There were 28 children in care aged 2 years-12 years with three staff present. Children were active in play groups, homework, activities, snack time, and rest time.
A Code inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in the Code standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. This inspection began at 2 PM and concluded at 4:30 PM.

Violations:
Standard #: 22VAC40-665-790-G
Description: Based on observation of snack, the center failed to ensure that all tables were sanitized and cleaned as required.

Evidence:
1. Observed in the preschool area the tables were not sanitized before and after eating snack.
2. Observed the school age children and the tables were not sanitized before eating snack.

Plan of Correction: The director will make sure that all staff are aware of the requirement for cleaning and sanitizing the tables.

Standard #: 22VAC40-665-620-A
Description: Based on observation, the vendor failed to ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to children. Cleaning supplies to clean and sanitize the diapering area or toilet chairs do not need to be kept locked during diapering or toilet training time as long as they are inaccessible to children.

Evidence:
1. In the preschool bathroom there was a spray can of pesticide that was unlocked in the medicine cabinet. The cabinet was approximately 3 1/2 feet off the ground.
2. In the ladies restroom in the school age area there were two cleaning spray bottles sitting on the window seal that was unlocked and accessible to the children.

Plan of Correction: The director will make sure all chemicals are locked. The space is shared space and will work with the church as well as staff.

Standard #: 22VAC40-665-780-A-1
Description: Based on review of the evacuation drills, the vendor failed to ensure that all drills were documented.

Evidence:
1. The September 2019 drill was not documented on the form.

Plan of Correction: The director stated that the drill was completed with the fire alarm company and was not documented on the form. In the future all drills will be documented as they occur.
