Comments:
An unannounced monitoring inspection was conducted Wednesday, November 13, 2019 from approximately 9:15 AM until 12:00 PM to determine the center's compliance with religious exempt requirements. A total of 68 children were present during the inspection and the following staff to child ratios were observed:
Ones: 7 children and 2 staff
Twos: 6 children and 2 staff
Threes: 12 children and 2 staff
Threes: 12 children and 2 staff
Fours: 17 children and 2 staff
Fours: 14 children and 2 staff
Children were observed creating spiders, playing with baby dolls, playing in the sensory table, creating turkeys, singing songs, playing with play-doh, doing stamp art, building with legos, learning about the letter ?Ii?, having books read to them, participating in circle time, washing hands and taking bathroom breaks and eating snack. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Ten children?s records and 10 staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Standard #: 22VAC40-191-40-D-2
Description: Based on review of 10 staff records, the center did not ensure that all staff and volunteers had a search of the central registry within the first 30 days of employment or volunteering.
Evidence:
1. The records for Staff #1 (hire date 09/18), Staff #2 (hire date 09/19), Staff #3 (hire date 09/14), Staff #4 (hire date 09/15), Staff #5 (hire date 09/18), Staff #6 (hire date 09/16), Staff #7 (hire date 09/15) and Staff #10 (hire date 09/04) did not contain central registry results.
2. A member of management stated she thought the criminal record check replaced the need to complete a central registry finding.

Plan of Correction: We will complete the central registry forms and send them to the agency. A copy will be sent to the licensing inspector within 10 days, showing proof that we requested the background check.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of five staff records, the center did not obtain updated background checks at least every five years for three staff members.
Evidence:
1. The record for Staff #3 (hire date: 09/14) contained a sworn statement dated 09/25/14.
2. The record for Staff #8 (hire date: 09/08) contained a central registry dated 05/14/08 and a sworn statement dated 04/24/08.
3. The record for Staff #9 (hire date: 09/04) contained a central registry dated 05/12/04.
4. The record for Staff #10 (hire date 09/14) contained a sworn statement dated 09/03/14.
5. A member of management stated she was not aware background checks were required to be updated at least every five years.
6. Effective July 1, 2017, staff at all religiously exempt child day centers must repeat background checks every five years.

Plan of Correction: The sworn statements were updated during the inspection. The central registry forms will be completed and sent to the local agency. Proof of the request will be sent to the licensing within 10 days.
