Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated regulations in child/staff ratios, CPR certification, out of state searches, and accessing the department's website for notices, forms, and additional information. A copy of the new regulations was given to the director during this inspection.

Comments:
An unannounced monitoring inspection was conducted today, November 13, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 10:50am and concluded at approximately 12:30pm. Today approximately fifty preschool children were in attendance being supervised by ten staff with the correct child/staff ratio in each classroom. Administrative records (including five staff and five children's records), children's classrooms, restrooms, fellowship hall, chapel, and common areas were inspected. Due to previous weather the playground was not inspected. See attached notice for violations found during this inspection.

Description: Based on the inspection of staff records, the required central registry background check was not conducted as required.

Evidence:
During the inspection of five staff records, it was discovered staff #2 did not obtain a central registry background check. The central registry background check was previously submitted to the correct agency; however, it was returned as additional information was needed. Religious Exempt requirements stipulate all applicants for employment, employees, applicants to serve as volunteers, and volunteers and any other person who is expected to be alone with one or more children enrolled in the child day center are to obtain the required central registry background checks.

Plan of Correction: Per director: I will have the form updated by the staff and immediately mail it back.
