The three background checks required for staff are 1. Federal Finger print based Criminal Record Check 2. Search of the Central Registry 3. Sworn Statement or Affirmation Each of these forms should be updated within 5 years of the last reported results. Please send documentation that the unmet requirements pertaining to background checks have been corrected within 10 days of the inspection. Corrections can emailed, faxed, or delivered in person to the Licensing Office. Comments:

A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on November 13, from 12 :20 p.m. to At the time of arrival the following staff to child ratios were observed: 13 children, four years old, with 2 staff 7 children, three to four years old, with 1 staff Classroom observations were made and children were observed settling onto mats for rest period and resting quietly. Five staff records were reviewed, posted information was reviewed and the center program administrator was interviewed regarding center policies and procedures. The facility, to include all indoor areas used by children in care, as well as the outdoor play space, were observed. Staff report that the program does not transport regularly children in a vehicle while in care. However, when attending planned occasional field trips children are transported in a school bus by one of two staff who have a commercial driver?s licenses (CDL). Description:	Based upon a review of records and interview, the religious exempt facility did not ensure that every employee, volunteer, or any other person who is expected to be alone with one or more children enrolled in the religious exempt child day center except a parent-volunteer, or a parent or guardian who may be left alone with his or her own child completes a Search of Central Registry background check and has the results of such within 30 days of employment or commencement of service. Evidence: Five staff records were reviewed. The record for staff #4 whose date of employment was documented as 8/20/19 did not contain the results of a Search of the Central Registry. The administrative assistant stated that she did not have this background check because her understanding was that the new federal background check had replaced the central registry search.
Plan of Correction:	The administrative assistant stated that she will have staff complete the Search of the Central Registry form, have it notarized and mailed out by Monday. A copy of the sent search request will be forwarded to the inspector and results will be forwarded to the inspector once they are received via email.
