An unannounced monitoring inspection was conducted Wednesday, November 13, 2019 from approximately 12:45 PM until 4:00 PM to determine the center's compliance with religious exempt requirements. A total of 106 children were present during the inspection and the following staff to child ratios were observed:
Little Saints: 14 children and 2 staff
JK: 14 children and 2 staff
JK: 12 children and 2 staff
Kindergarten: 18 children and 3 staff
First grade: 11 children and 3 staff
First grade: 10 children and 3 staff
Second grade: 8 children and 2 staff
Second grade: 12 children and 3 staff
Second grade: 7 children and 2 staff
Children were observed napping, looking at books, playing outside, building with blocks, and completing homework. All areas of the center were inspected including the Little Saints, JK and Extended Day classrooms, hallways, bathrooms and outdoor play areas. Ten children?s records and 10 staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of two staff records and interview, the center did not obtain satisfactory results of the fingerprint-based national criminal background check from the department prior to employment for seven staff members.
Evidence:
1. The record for Staff #1 contained a criminal record check dated 05/13/19.
2. The record for Staff #4 contained a criminal record check dated 05/13/19.
3. The record for Staff #5 contained a criminal record check dated 10/19/18.
4. The record for Staff #6 contained a criminal record check dated 10/22/18.
5. The record for Staff #7 contained a criminal record check dated 08/23/18.
6. The record for Staff #8 contained a criminal record check dated 08/23/18.
7. The record for Staff #9 contained a criminal record check dated 05/13/19.
8. The criminal record checks on file were completed directly by the State Police and Federal Bureau of Investigation United States Department of Justice, and the results were sent to the school directly from the State Police.
9. Fingerprint-based national criminal background checks obtained after 01/21/2018 are required to be completed through the department?s vendor, Fieldprint, so the results come directly from the Virginia Department of Social Services .Plan of Correction: Not available online.
