An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted November 12, 2019 from 8:30 a.m. to 10:45 am. There were 44 children, ages 2 years through 5 years old, were under the care and supervision of 11 staff. Center documents and 13 staff records were reviewed.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of documentation and interview with staff, it was determined that the center did not obtain sworn statement or affirmation before employment and central registry background checks within 30 of employment. Evidence: Staff A's (start date: 8/2017) sworn statement was dated 8/14/2019 and central registry check was dated 9/9/19. Staff B did not have documentation of a sworn statement in their record. Plan of Correction: All staff members have current sworn statements and central registry background checks in the record. Moving forward the facility will ensure that all employees have a sworn statement and central registry check completed within the time frames.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of documentation and interview with staff, it was determined that the center did not obtain an out of state child abuse and neglect search for all staff within 45 days of employment. Staff C (start date 8/20/18) did not have documentation of an out of state child abuse and neglect search request in their record. Plan of Correction: The center will complete an out of state child abuse and neglect search request for the staff.
