Technical Assistance:
All background checks (sworn statement or affirmation, fingerprints and search of the central registry) must be updated every five years.

Please remember that you need to forward the results of the missing background checks to me upon receipt.

Comments:
Licensing Inspector arrived at 10:30 and departed at 11:15 AM to conduct a code compliance inspection for religiously exempt child day centers. At the time of the inspection, there were 11 preschool aged children in care with two staff. A private school also operates at this facility. Children were observed eating snack and playing outdoors. The sample size was two staff records.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of two staff records, it was determined that the facility did not ensure that all staff had a completed search of the central registry on file within 30 days of employment.

Evidence:
The record for staff #2 did not contain a completed search of the central registry. Staff #2 began employment at the beginning of the school year.

Plan of Correction: Office manager will send for search of the central registry and place results in the record.

Standard #: 63.2(17)-1716-A-6
Description: Based on review of handbook provided by office manager during inspection, it was determined that the facility did not ensure that all required aspects were described in a document made available to parents, guardians and the general public.

Evidence:
There was no information in the hand book regarding the physical facilities or enrollment capacity of the center.

Plan of Correction: Information will be added to next update of the handbook for parents and guardians.

Description: Based on review of two staff records, it was determined that the facility did not ensure that all background checks were updated every five years.

Evidence:
The most recent search of the central registry for staff #1 was dated 12/23/08 and was, therefore over five years old

Plan of Correction: Office manager will send for search of the central registry and place results in the record.
