Licensing Inspector arrived at 9:15 and departed at 10:00 AM to conduct a code compliance inspection at a religiously exempt child day center. At the time of the inspection, there were 20 children, ages two through four years, in care with four staff. Children were observed participating in self-directed activities. The sample size was five staff records.
- Violations:

Standard #: 22VAC40-191-40-D-2 Description: Based on review of five staff records, it was determined that the facility did not ensure that all staff had required background documents (sworn statement or affirmation & search of the central registry) on file within prescribed time frames.
Evidence:
1. The records for staff #1 and staff #2 did not contain sworn statements or affirmations. Both staff were observed on duty with the children during the inspection.
2. The records for staff #4 and staff #5 did not contain results of completed searches of the central registry. Both staff have been employed over 30 days.Plan of Correction: 1. Staff will sign sworn statement or affirmations and they will be placed in the staff records. In the future, these documents will be signed and filed prior to employment.
2. All searches of the central registry have been sent for. When results are received, they will be placed in staff member's records.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of five staff records, it was determined that the facility did not ensure that all staff completed fingerprinting as required within the prescribed time frames.
Evidence:
The record for staff #3 did not include completed fingerprints.Plan of Correction: Center management will either locate fingerprints and place in record or have staff member complete fingerprints again. In the future, all staff will have completed fingerprints on file prior to employment.
