A self-reported incident was received in the Central Regional Licensing office on October 24, 2019 concerning a playground injury. The licensing inspector conducted an unannounced other inspection in response to the self-reported incident on November 18, 2017 at 11:15 am. Interviews with staff were conducted, observations made inside/outside the center, and documentation related to the incident were reviewed. Additional information was obtained on November 21, 2019 off-site. Based on the information gathered during the investigation, there were no violations cited related to the self-reported incident.
