Licensing Inspector arrived at 10:30 AM and departed at 12:30 PM to conduct an annual subsidy health & safety inspection. At the time of the inspection, there were 12 children (ages two through five) in care with two staff. Children were observed doing table work and dancing. Lunch was also observed.

Standard #: 22VAC40-665-520-B Description: Based on review of five children's records, it was determined that the facility did not ensure that all required elements were present in each child's record.
Evidence:
The record for child #4 did not contain information for an emergency contact person.Plan of Correction: Center director will ensure that all elements are present in each staff member's file.
Standard #: 22VAC40-665-560-A Description: Based on review of five children's records, it was determined that the facility did not ensure that all children had immunizations on file before the child can attend the center.
Evidence:
The record for child #1, who was in care at the time of the inspection, did not contain documentation of immunizations.Plan of Correction: Center director will contact parents to obtain copy of immunizations and place them in the record.
