Licensing Inspector arrived at 9:15 and departed at 10:30 AM to conduct a subsidy health and safety inspection in conjunction with a religiously exempt child day center code compliance inspection. At the time of the inspection, there were three two-year-old children in care with one staff. Children were observed watching television. The sample size was two staff and five children's records.

Standard #: 22VAC40-665-520-B-4 Description: Based on review of five children's records, it was determined that the vendor did not ensure that each child's record had the name, address and telephone number of at least on person designated by the parent to contact in case of an emergency.
Evidence:
The record for child #3, date of enrollment 10/18) did not contain information for an emergency contact person is case the parent cannot be reached.Plan of Correction: Center director will contact parent and have them supply information on an emergency contact person.
Standard #: 22VAC40-665-560-A Description: Based on review of five children's records, it was determined that the vendor did not ensure that all children had documentation of immunizations required before the child can attend the center.
Evidence:
The record for child #2, date of enrollment 9/24/18, did not include documentation of immunizations.Plan of Correction: Center director will contact the parents to obtain copy of immunizations.
Standard #: 22VAC40-665-780-A-1 Description: Based on review of documents at the center, it was determined that the vendor did not ensure that evacuation were practiced monthly.
Evidence:
Although there were two evacuation drills documented for November 2019, there was not an evacuation drill documented for December 2019.Plan of Correction: Center director will ensure that evacuation drills are held and documented monthly.
