An unannounced monitoring inspection was conducted on 1/29/20 from 10:00 am until 12:45 pm to review the subsidy health and safety requirements. At the time of the inspection 62 children were present with 15 staff. The sample size consisted of six staff's records, six children's records and six children's medications and supporting documents. Children and staff were observed during indoor and outdoor free play, educational activities, story time, bathroom break, diapering, hand washing, lunch, nap, transitions and behavioral management. Standard #: 22VAC40-665-520-B-7 Description: Based on record review and interview, the vendor failed to obtain documentation of an immunization record for one out of five children's records before the child's first day of attendance.
Evidence:
1. The record for child 2 did not contain documentation of immunizations. Child 2's start date was documented as 8/19/19.
2. The administrator verified child 2's start date and that they did not have an immunization record.Plan of Correction: The parents of child 2 will be advised to provide documentation of an immunization record. In the future no child will be allowed to attend the center until the immunization record is provided.
