An unannounced monitoring inspection was conducted on 1/29/20 from 10:00 am until 12:45 pm to review the religious exempt requirements. At the time of the inspection 62 children were present with 15 staff. The sample size consisted of six staff's records, six children's records and six children's medications and supporting documents. Children and staff were observed during indoor and outdoor free play, educational activities, story time, bathroom break, diapering, hand washing, lunch, nap, transitions and behavioral management. Standard #: 63.2(17)-1716-A-4 Description: Based on record review and interview, the center failed to obtain documentation from a practicing physician stating each staff is free from any disability which would prevent him from caring for children under his/her care prior to working with children.
Evidence:
1. The records of six staff were reviewed. The following two staff did not have a staff health report prior to working with children:
Staff 2's start date 8/1/19 and staff health report dated 8/26/19.
Staff 5's start date 5/2/19 and staff health report dated 6/7/19.
2. The administrator verified the start dates and staff health report dates.Plan of Correction: In the future, no new staff will be allowed to work with start work until the staff health report is completed.
Standard #: 63.2(17)-1716-B-4 Description: Based on record review and interview, the center failed to implement a procedure to ensure that all children in the center are in compliance with immunization provisions.
Evidence:
1. Six childrens' records were reviewed. Child 2's record did not contain an immunization record. The start date for child 2 is documented as 5/7/19.
2. The center's procedure is for immunizations to be obtained prior to attendance.Plan of Correction: In the future the administration will ensure no child will start attending the center until immunization records have been provided.
