A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on January 28, 2020, from 1:40 p.m. to 2:25 p.m. Staff records were reviewed on 1/30/2020 from 10:30 a.m. to 11:30 a.m.
At the time of arrival on 1/28/2020 the following staff to child ratios were observed:
5 children, 18 months to 3 years of age with 2 staff.
Children were observed playing with a variety of toys in a mixed group in one room for extended care following the regular preschool program.
Five staff records were reviewed, posted information and parent handbooks were reviewed and the center program?s Director was interviewed regarding the centers policies and procedures. The facility, to include all indoor areas used by preschool children and before and after care children, as well as the outdoor play space, were observed.
Staff report that the program does not routinely transport preschool children in a vehicle.

Standard #: 22VAC40-191-40-D-2 Description: Based on a review of records and interview, the religious exempt program did not obtain the results of a search of the central registry for all staff within 30 days of employment.
Evidence:
Five staff records were reviewed.
The records for staff #1 (employed since 9/2012) and staff #5 (employed since 9/2018) did not contain the results of a Search of the Central registry.
Both records contained documentation that the initial requests for the Search for the Central Registry were sent on 11/21/18 and returned due to errors on the request form. Documentation showed that the forms were resent with corrections on 11/30/18. However, there was no documentation of the results having been received and no documentation that the program had followed up with the agency to inquire about the results since then.Plan of Correction: The program contacted the agency during the inspection and is taking steps to obtain the results. Contact will continue with the agency and documented, until the results are obtained.
