Comments:
An unannounced monitoring inspection was conducted on Monday, February 24, 2020 to determine the center's compliance with religious exempt requirements. The licensing inspector was on site from approximately 10:45 AM until 1:45 PM. The following ratios were observed with a total of 52 children present during the inspection:
Twos - 9 children with 2 staff
Threes - 17 children with 2 staff
Fours - 14 children with 2 staff
Fours - 12 children with 2 staff
School age children were not present during the inspection.

Children were also observed playing with baby dolls, throwing a ?Birthday Party? in dramatic play, playing on a sit and spin, learning about the letter ?Qq?, playing outside, taking bathroom breaks and washing hand. All areas used by children were inspected to include the classrooms, hallways, bathrooms and outdoor play areas. During the inspection, six children's records and six staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. A violation was cited during the inspection. Description: Based on record review and interview, the center failed to ensure two new staff have been certified by a practicing physician to be free from any disability which would prevent him from caring for children by the first date of employment.

Evidence:
The record of Staff #1 has a physician form dated 02/14/2020. Staff #1 was first date of working in the center was 02/03/2020. A member of management confirmed this information during the inspection.

Plan of Correction: We will not allow future employs begin working until the Staff Health Report is submitted.
