Comments:
On December 19, 2019, the Department received a self-reported concern for an incident that occurred on December 19, 2019 regarding supervision. The licensing inspector and a representative from a local agency were on site conducting an unannounced inspection on December 30, 2019 from approximately 10:00 AM until 11:35 AM and on January 2, 2020 from approximately 3:00 PM until 4:00 PM. While on site, interviews with staff were conducted, observations were made and documentation was reviewed. Additional interviews were conducted on January 28, 2020. Based on the information gathered during the investigation, the preponderance of evidence supports a violation of the statutes.


Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 63.2(17)-1716-B-2
Description: Based on interviews and a review of documentation, the center did not establish and implement procedures for appropriate supervision of all children in care, including daily intake and dismissal procedures to ensure safety of children.

Evidence:
During the December 30, 2019 and January 2, 2020 inspections, the following was determined:
1. Two staff stated on December 19, 2019 between approximately 7:05 AM to 7:20 AM, Child #1 (three years of age) was left on the center?s bus.
2. Staff #1 reported on the morning of December 19, 2019, she picked up children on her morning bus run and arrived at the center between 7:05 AM and 7:10 AM. Staff #1 stated, "I was running late. There was some chaos at the center because of an angry parent. The parent was up front, and I went to get [Staff #2] to talk to the parent. Another staff said [Staff #1] you left a kid on the bus?.
3. Staff #2 stated ?[Witness #1] and I went outside to the bus and [Child #1] was alone on the bus, crying. The bus was parked in front of the building and the doors on the bus were left cracked open. [Witness #1] opened the door and got [Child #1] off the bus. I know it was after 7:19 AM because [Witness #1] clocked her child in at the center at 7:19 AM.
4. The transportation attendance log for the morning of Thursday December 19, 2019 indicated there were seven children being transported on the bus. Child #1 was documented as getting on the bus at 6:41 AM.
5 A member of management stated the driver is supposed to walk to the back of the bus and push the button that turns off the alarm to ensure everyone is off the bus. Staff #1 stated, ?One of the kids that sat in the back seat pushed the button?.
6. The local weather temperature for the morning of December 19, 2019 was approximately between 28 and 32 degrees Fahrenheit.

Plan of Correction: Each driver has a transportation log for home/school morning pickup from home. Departure time from center is recorded. An X is placed if the child is absent from the bus/van. Children are placed on bus/van (older in back and younger in the front). Morning Arrival at the Center. Driver counts children when they arrive. Management receives count from dicer as they receive children. Driver hits the button and walks the bus from back to front to ensure no children are left on the bus/van. Management double checks the bus/van before driver secures the bus/van. Van/Bus ride to school. Driver count children before they get on the bus/van. Children are counted as they arrive at each school. Arrival back to center after morning school run. Driver hits button and walks from back to front to checks for children. Management double checks bus/van for children before driver secures bus/van. Van/Bus ride from school. A count is done of children picked up from school. Management gets count and receives children. Driver hits the button and walks from back to front, checking the bus for children. Management double checks bus/van for children before driver secures the bus/van. Evening drop off and back to the center. Driver and management count children riding home. Driver writes down the time each child arrive home. Driver writes down time they arrived back to the center. Driver hits the button and walks from back to front checking the bus for children. Management double checks the bus/van for children before the driver secures the bus. Management and drivers will be responsible implementing this plan.
