Comments:
On December 19, 2019, the Department received a self-reported concern for an incident that occurred on December 19, 2019 regarding supervision. The licensing inspector and a representative from a local agency were on site conducting an unannounced subsidy inspection on December 30, 2019 from approximately 10:00 AM until 11:35 AM and on January 2, 2020 from approximately 3:00 PM until 4:00 PM. While on site, interviews with staff were conducted, observations were made and documentation was reviewed. Additional interviews were conducted on January 28, 2020.

Based on the information gathered during the investigation, the preponderance of evidence supports violations of subsidy health and safety requirements.
