22VAC40-665 INTRODUCTION

An unannounced monitoring inspection took place on February 26, 2020. There were 27 children in care from ages 19 months old to 5 years old with 6 staff providing care and supervision. The inspector reviewed children's health information and 11 staff records during the inspection. Per the staff, the center does not administer medications or transport children. Children were observed eating a snack and engaging with staff. The inspector also reviewed policies and procedures. The inspection took place from approximately 9:30 AM to 12:00 PM. Standard #: 22VAC40-191-40-D-2 Description: Based on record review and discussion with staff, the center failed to ensure that background checks were obtained as required by the Code of Virginia.
Evidence:
1. Staff 1's record did not contain a Sworn Disclosure. Staff 1 verified that there was no Sworn Disclosure and completed a Sworn Disclosure during the inspection.
2. Staff 6's record did not contain a Sworn Disclosure. Staff 1 verified that there was no Sworn Disclosure Statement completed.Plan of Correction: Staff will update all background checks.
Standard #: 63.2(17)-1716-A Description: Based on observation and discussion with staff, the center failed to ensure that a statement of intent to operate a child day center, certification that the child day center has disclosed in writing to the parents or guardians of the children in the center is posted in a visible location on the premises.
Evidence:
1. When asking Staff 1 where the Statement of Intent was posted, Staff 1 stated that it was located in a file.Plan of Correction: Staff will post this information in a conspicuous location.
Standard #: 63.2(17)-1716-A-6 Description: Based on record review and observations, the center failed to ensure that all aspects of the child day center's operations are described in a written statement provided to the parents or guardians of the children in the center and made available to the general public.
Evidence:
1. Physical facilities, enrollment capacity, food services, and health requirements for staff where not described in a written statement provided to the parents or guardians of the children in the center.Plan of Correction: Staff will post this information until the next year's handbook is compiled.
Standard #: 63.2(17)-1720.1-A Description: Based on record review and discussion with staff, the center failed to ensure that background checks were completed as required by the Code of Virginia.

1. Staff 5's record contained a Central Registry Search dated 10/03/2006. Staff 1 verified this information.Plan of Correction: Staff will ensure that this background check is updated.
