Comments:
An unannounced monitoring inspection was conducted on March 4, 2020 from approximately 9:24 am- 11:30 am. Upon arrival to the facility, there were six staff members present with 28 children. Three classrooms with restrooms in each room were inspected. The outdoor playground area and equipment were reviewed. Licensing inspector reviewed administration, the physical environment and supervision. The children were observed arriving to the program, having open circle and singing songs and listening to a lesson in Chapel led by the Pastor. Eight staff records were reviewed. The facility does not offer transportation.

Violations:
Standard #: 63.2(17)-1716-A-4
Description: Based on record review, in two of eight staff records reviewed, the center did not ensure that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his/her supervision.

Evidence: The Director confirmed that there was not a current annual staff health report for the following staff members: Staff #7-most recent staff health report dated 6/4/18 and Staff #8-most recent staff health report dated 1/28/19

Plan of Correction: Director will have Staff #7 and Staff #8 to go to the doctor to obtain a current health report. The Director will keep an eye on the dates for all employees.

Standard #: 63.2(17)-1716-A-6
Description: Based on review and staff interview, the center did not describe all required aspects of the child day center's operations in a written statement to the parents/guardians of the children in the center and made available to the general public.

Evidence: The Director confirmed that the health requirements for staff had not been provided in writing to the parents/guardians and made available to the general public.

Plan of Correction: Director will add it to the preschool handbook for next school year. An amendment will be sent home to all families to add to the current handbook.
