Technical Assistance:
The licensing inspector provided consultation regarding 22VAC40-191-(BC)-60 regarding consequences unsatisfactory results of background checks. Consultation was also provided regarding 22VAC40-665-(9)-740-A-2 stating children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine and 22VAC40-665-(9)-740-A-4 stating if running water is not available, a germicidal cleansing agent administered per manufacturer's instruction may be used..

Comments:
An unannounced subsidy health and safety inspection was conducted with the director beginning at 10:30 AM and ending at 2:00 PM. There were eighty-eight children in care during the inspection. The children's ages ranged between five months and four years old. The infants were observed sleeping in cribs and playing with soft toys. The older children were observed during instructional time, watching an age appropriate movie, and being read stories by teachers. Ten children's records were reviewed. Five staff records were reviewed and in compliance. The most recent emergency escape drill was documented as 2/14/20. The most recent shelter-in-place drill was documented as 4/9/19. The most recent lockdown drill was documented as 3/19/19. The most recent fire inspection was dated 6/3/19. The most recent health inspection was dated 5/17/19.

Violations:
Standard #: 22VAC40-665-610-A
Description: Based on observation, not all of the equipment in the center was maintained in a safe condition. Evidence: In the infant room the dvd was being stored on the window sill in an unstable manner. The cords from the television mounted on the wall were accessible to children. The Insignia brand television in the K4B classroom was not anchored to the television stand presenting a crushing hazard.

Plan of Correction: The vendor stated that the dvd player would be relocated to another area. The vendor stated that the flat screen televisions in the K4B classroom was going to be mounted to the wall.

Standard #: 22VAC40-665-610-C-2
Description: Based on observation, the vendor did not ensure that all the electrical outlets had protective covers. Evidence: There were two outlets in the K4A classroom that did not have protective covers.

Plan of Correction: The vendor will ensure that all outlets have covers on them.

Standard #: 22VAC40-665-650-A
Description: Based on observation, the vendor did not ensure that there was one staff member for every 10 children ages three to five years old. Evidence: The K3B class had twenty-one children in the care of two staff members.

Plan of Correction: One of the children will be reassigned to a different classroom. The vendor will ensure that the appropriate ratio is maintained per their age level.

Standard #: 22VAC40-665-710-H
Description: Based on observation, the vendor failed to ensure that all full-size cribs have mattresses that fit snugly next to the crib so that no more than two fingers can be inserted between the mattress and the crib. Evidence: There were three cribs in the infant room that had two resting mats stacked in them; the mats were being used as crib mattresses.

Plan of Correction: The vendor stated that mats would be removed and crib mattresses would be purchased and used moving forward.

Standard #: 22VAC40-665-740-B-8
Description: Based on observation, the vendor failed to ensure there was a diapering surface and that the diapering surface was nonabsorbent. Evidence: In the infant classroom, the diapering pad had a rip exposing the cushioning material making it absorbent. In K2 classroom, the diapering pad had a pink absorbent cover on it.

Plan of Correction: The cover will be removed from the changing pad.
