Serious injury procedures when a child requires outside medical treatment

An unannounced monitoring inspection was conducted on March 10, 2020 from approximately 9:55 am-12:49 pm. Upon arrival to the facility, there were 26 children present with six staff members. The children were observed during circle time, center time and during outdoor play. The inspector reviewed the indoor and outdoor areas, background checks and administration. Eight staff records were reviewed. Three classrooms and three restroom areas were inspected. The center does not offer transportation.

Standard #: 63.2(17)-1716-A-4 Description: Based on record review, in one of eight staff records reviewed, the center did not ensure that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his or her supervision.
Evidence: The Director confirmed that the most recent staff health report for Staff #6 was more than one year old and was dated 12/28/18Plan of Correction: I have notified the staff member to go to the doctor and update the form. She contacted her doctor he will update the form accordingly. The date to be completed will be before we reopen to the public which is April 3, 2020
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, in one of eight staff records reviewed, the center did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him.
Evidence: Staff #4 indicated as to living outside of the state of Virginia on a sworn statement completed on 12/2/18. The Director confirmed that there was no documentation that an out of state child abuse and neglect search requested on 11/1/19 was followed up within 45 days after the request. There were no results of an out of state search for this staff member.Plan of Correction: I mailed an application to California to obtain a copy of state summary criminal history for YPP employee #4 on 11/11/2019. I did not receive anything back. I called the number on the form: California Department of Justice (916) 227-3885. I got an automated message that directed me to call (916) 227-2300 to get updates on the processing of such applications. The phone rang and never sent me to voicemail. The California Department of Justice may be closed due to Cornona-Virus. My plan moving forward is to call weekly and ultimately obtain her history or a written statement that they will not release.
