An unannounced monitoring inspection was conducted on Thursday, March 12, 2020 to determine the center's compliance with religious exempt requirements. The licensing inspector was on site from approximately 1:15 PM until 3:15 PM. The following ratios were observed with a total of 22 children present during the inspection:
Twos - 5 children with 1 staff
Threes and Fours - 7 children with 1 staff
Pre-K - 10 children with 2 staff
Children were also observed resting at naptime, playing with toys, taking bathroom breaks, washing hands and eating snack. All areas used by children were inspected to include the classrooms, hallways, bathrooms and outdoor play areas. During the inspection, five children's records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. The results of the inspection were reviewed during an exit interview with a member of management. A violation was cited during the inspection. See the violation notice for additional information. Standard #: 63.2(17)-1720.1-B-3 Description: Based on the review of the staff five records and interview, the center did not request a search of out-of-state founded complaints of child abuse and neglect within the first 30 days of beginning employment for one staff member who has lived outside of Virginia within the last five years.
Evidence:
The record for Staff #4 (hire date 02/04/19) indicated the staff member had lived outside of Virginia within the last five years. The record did not contain documentation of a request for founded complaints of child abuse and neglect from the state the staff member indicated she had lived in within the last five years. A member of management stated the information has not been requested as of 03/12/2020.Plan of Correction: We will request the information and send documentation of the request to the licensing inspector within the next 10 days.
