Comments:
Licensing Inspector arrived at 10:00 and departed at 11:30 AM to conduct a subsidy health and safety inspection in conjunction with a religiously exempt code compliance inspection. At the time of the inspection, there were 19 children (ages infant through preschool) in care with four staff. Children were observed watching educational movies, participating in large motor activities and eating lunch.

Violations:
Standard #: 22VAC40-665-790-C
Description: Based on observation it was determined that the vendor did not ensure that when food is brought from home it will be labeled with the owners name and the date.

Evidence:
Upon arrival, licensing inspector noticed several infants and toddlers drinking from bottles. None of the bottles were labeled with name and date.

Plan of Correction: Staff will ensure that all bottles are labeled and dated when brought into the facility.

Standard #: 22VAC40-665-530-2-a-1
Description: Based n a review of six staff records, it was determined that the vendor did not ensure that all staff had completed fingerprinting as required.

Evidence:
1. Staff #5 was observed on duty with the children during the inspection but did not have fingerprints on file. . Center management stated that staff #5 had been employed "about two weeks."
2. Staff #1 had fingerprints on file dated 11/6/19. Staff #1 stated that she had begun work in August of 2019.

Plan of Correction: Center management will ensure that checks are completed and results placed in the record. MAnagement will ensure that all persons will obtain fingerprints prior to employment.

Standard #: 22VAC40-665-530-2-a-2
Description: Base on a review of six staff records, it was determined that the vendor did not ensure that all staff had a completed search of the central registry (CPS check) on file.

Evidence:
The records for staff #1 (date of employment 8/19) and staff #2 (date of employment 5/19) did not contain the results of a search of the central registry

Plan of Correction: Center management will ensure that checks are completed and results placed in the records.

Standard #: 22VAC40-665-540-A
Description: Based on a review of six staff records, it was determined that the vendor did not ensure that each staff had documentation of a tuberculosis (Tb) screening submitted at the time of employment that had been completed within the last 30 calendar days.

Evidence:
The records for staff #2 and staff #5, both of whom were identified as staff members, did not have documentation of completed Tb screenings.

Plan of Correction: Center management will ensure that all staff obtain Tb screenings. Additionally, center management will ensure that staff obtain Tb screenings within the prescribed time frames.

Standard #: 22VAC40-665-540-B
Description: Based on review of six staff records, it was determined that the vendor did not ensure that all staff had updated tuberculosis (Tb) screening every two years from the date of the last screening.

Evidence:
1. The record for staff #6 contained a Tb screening dated 1/9/18 and was, therefore over two years old.

Plan of Correction: Center management will ensure that all staff obtain updated Tb screenings in a timely manner.
