-This annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.
Preliminary data was obtained from the vendor on 4/27/20.
-An annual subsidy inspection was initiated on 4/29/20, at which time the vendor was contacted by telephone to initiate the inspection, and concluded on 4/29/20. The inspector emailed the vendor a list of items required to complete the inspection.
The vendor reported there were 19 children in care. The inspector reviewed 4 children?s records and 2 staff member records submitted by the vendor as required by the health and safety regulations.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Standard #: 22VAC40-665-520-B-7 Description: Based on record review, the vendor did not ensure one of four children's records contained documentation of immunization records on or before the child's first day of attendance.
Evidence:
The record of child #4 (start date: 6/12/17) did not have documentation of immunization records.Plan of Correction: I can ask the parent's to get a shot record.
Standard #: 22VAC40-665-530-2-a-2 Description: Based on record review, the vendor did not ensure two of two staff records had documentation of satisfactory central registry results within 30 days of employment.
Evidence:
1. The record of staff #1 (start date: 1/4/16) did not have documentation of satisfactory central registry results.
2. The record of staff #2 (start date: 8/24/18) did not have documentation of satisfactory central registry results.Plan of Correction: I will ask administration to see where the documents are.
