2/3/20 - Licensing Inspector arrived at 9:00 and departed at 10:30 AM to interview staff in relation to a significant incident involving staff and child interaction. Three staff and members of management were interviewed regarding the incident.

Standard #: 63.2(17)-1716-B-2 Description: Based on review of video tape footage as well as interviews with staff and management, it was determined that the facility failed to implement appropriate supervision procedures.
Evidence:
1. On January 21, 2020 a staff #1 was observed striking child #1 (age three) in the face with a rolled up jacket. She then grabbed the same child up with both hands on the lower arm, picking him up so that the child's feet were off the floor and moved him to another location in the room.
2. Staff stated in interviews that child #1 did not cry after the incident. However, child is seen holding his right arm and not participating in the music activities that were going on after the incident.
3. Child #1 told his father upon arrival that his arm hurt. He was later diagnosed with a dislocated elbow (nursemaid?s elbow). Child #1 told his father that staff #1 pulled his arm.
4. Several minutes later, staff #1 was observed grabbing another three-year-old child by the lower arm and forcefully walking him across the room. She then picked this child up so his feet were off the floor and placed him in a chair.
5. Staff #1 stated that the classroom was stressful and that she ?felt stress and a little frustrated.?Plan of Correction: The facility responds with:
As soon as the incident was reported, the staff member was removed from the center and since been terminated.
All staff members have received behavioral guidance and forbidden actions mandated by the State of Virginia as a review. We also reviewed signs of abuse and neglect and reviewed that we are required to report any suspected child abuse and neglect.
We have also implemented smaller groupings of children and trained on how to handle a strong willed child. All the review and training was done by the Director and Assistant Director. We will continue to monitor classrooms and offer guidance.
