An unannounced Subsidy Health and Safety inspection was conducted on March 12, 2020 beginning at 9:30 a.m. to 1:50 p.m. with the director. There were thirty children, ranging in age from four months to eight years old, in the care and supervision of five staff. Children were observed during nap time. Center documents and five children's records were reviewed and in compliance. Seven staff records were reviewed. There were two medications on site. Two staff were Medication Administration Training (MAT) certified. The emergency escape and shelter in place drill logs were reviewed and in compliance. The most recent lock down 3/19/19.

Standard #: 22VAC40-191-40-C-2 Description: Based on review of documentation, the facility failed to obtain sworn statement of affirmation by the first day of hire and Central Registry Background checks within thirty days of hire. Evidence: Staff A, date of hire 11/12/2019, did not have documentation of Central Registry Background check results. Plan of Correction: The central registry request was filled out and in the staff record; but it was not mailed off. The central registry request will be mailed off immediately.
Standard #: 22VAC40-665-530-3 Description: Based on a review of seven staff records, not all of the staff members had documentation of a current tuberculosis. Evidence: Staff B's most recent tuberculosis screening was dated 2/8/19. Plan of Correction: The staff member is already scheduled for a doctors appointment and the updated screening will be placed in the staff record once received.
