Comments:
This annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.
Preliminary data was obtained from the vendor on 6/30/2020.
An annual subsidy inspection was initiated on 7/01/2020, at which time the vendor was contacted by telephone to initiate the inspection, and concluded on 7/7/2020. The inspector emailed the vendor a list of items required to complete the inspection.
The vendor reported there were 36 children in care. The inspector reviewed 3 children?s records and 3 staff records submitted by the vendor to ensure compliance with the health and safety regulations.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for
Child Care programs.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Violations:
Standard #: 22VAC40-665-530-2-a-2
Description: Based on record review, the center did not ensure three of three staff records contained documentation of satisfactory results of the Child Protective Services Central Registry check.
Evidence:
1. The record of staff #1 (start date: 5/29/18) did not have documentation of Child Protective Services Central Registry results.
2. The record of staff #2 (start date: 5/22/18) did not have documentation of Child Protective Services Central Registry results.
3. The record of staff #3 (start date: 7/15/19) did not have documentation of Child Protective Services Central Registry results.

Plan of Correction: We will mail off central registry forms today and will follow up next week.

Standard #: 22VAC40-665-580-B
Description: Based on record review, the center did not ensure three of three staff records contained documentation that they complete the Virginia Pre-service Training.
Evidence:
1. The record of staff #1 (start date: 5/29/18) did not have documentation of the Virginia Pre-service Training.
2. The record of staff #2 (start date: 5/22/18) did not have documentation of the Virginia Pre-service Training.
3. The record of staff #3 (start date: 7/15/19) did not have documentation of the Virginia Pre-service Training.

Plan of Correction: We will have all staff complete the training by August 7th.

Description: Based on record review, the center did not ensure one of three staff records contained documentation that the staff has current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

The record of staff #2 (start date: 5/22/18) did not have documentation showing that the staff has certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Plan of Correction: All staff will complete first aid and cpr training.

Description: Based on record review, the center did not ensure one of three staff records contained documentation that the staff has current certification in first aid.
Evidence:
The record of staff #2 (start date: 5/22/18) did not have documentation showing that the staff has certification in first aid.

Plan of Correction: All staff will complete first aid and cpr training.

Standard #: 22VAC40-665-580-H
Description: Based on record review, the center did not ensure two of three staff records contained documentation showing that the staff completed the Annual Virginia Health and Safety Update Training by March 1, 2020.
Evidence:
1. The record of staff #1 (start date 5/29/18) did not have documentation of the Annual Virginia Health and Safety Update Training. The center's representative stated that the center was not aware of the new requirement.
2. The record of staff #2 (start date 5/22/18) did not have documentation of the Annual Virginia Health and Safety Update Training. The center's representative stated that the center was not aware of the new requirement.

Plan of Correction: We will have all staff take the training by August 7th.
