An unannounced Subsidy Health and Safety Inspection was conducted with the provider on 8/29/23 from 9:05am to 10:00am. There were no children in care and the provider reports no children are currently enrolled. The provider?s record and two household member records were reviewed. The program?s policies and procedures were discussed for when children are enrolled. Violations were found during the inspection and details can be found on the Violation Notice. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/15/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-790-250-D-1 Description: Based on review of documentation and interview, the provider did not ensure that all caregivers had current certification in cardiopulmonary resuscitation (CPR). Evidence: The provider?s CPR certification was completed on 5/15/21 and expired on 5/15/23. Plan of Correction: CPR re-certification is scheduled for 9/23/23.
Standard #: 8VAC20-790-250-F Description: Based on review of documentation and interview, the provider did not ensure that caregivers who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course. Evidence: The provider did not provide documentation of completing the preservice training, the health and safety update course, or certificates towards annual training.
