This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.
The department received a complaint incident regarding allegations in the areas of supervision and physical plant. A subsequent investigation was initiated on 9/1/2020 and concluded on 9/1/2020. The inspector contacted the vendor by telephone to conduct the investigation. The inspector reviewed documentation required to complete the investigation, interviewed additional staff and made observations via video call.
The evidence gathered during the investigation did not support the allegations of non-compliance with standards or law. Though not a part of the original complaint, there were violations found during the inspection regarding staff training. See the violation notice for more details.

Standard #: 22VAC40-665-580-H Description: Based on record review, the center did not ensure two of two staff records contained documentation showing that the staff completed the Annual Virginia Health and Safety Update Training by March 1, 2020.
Evidence:
1. The record of staff #1 (start date 1/1/99) did not have documentation of the Annual Virginia Health and Safety Update Training.
2. The record of staff #2 (start date 1/1/99) did not have documentation of the Annual Virginia Health and Safety Update Training.Plan of Correction: Both staff will have it finished by next Saturday.
