Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

The licensing inspector for Community Fellowship Church of the Nazarene conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 10/16/20. Interviews were conducted relating to allegations of lack of supervision of children. The information gathered during the investigation supports the allegation, so the complaint is determined to be "valid." Description: Based on interview, the center failed to ensure one staff member was present with every five children when the youngest child in the group was in the age range of 16 months to 24 months.

Evidence:

1. Staff 2 stated three staff were outside with what he/she thought was more than 16 children but staff 2 was told it was 16 children on 10/13/20 around 4:30 pm to 5:15 pm. The youngest child was around one and a half. At some point staff 1 went inside to conduct an interview. Staff 2 was unsure how long Staff 1 was inside.
2. Staff 3 stated on 10/13/20 three staff were outside with 16 children with the youngest being one and a half from approximately 4:30 pm until 4:45 pm when staff 1 went inside. At that time staff 3 and staff 2 were left with the 16 children until 5:15 when they took the children inside.
3. Staff 1 stated on 10/13/20 there were 16 children on the playground with the youngest being 21 months old.

Plan of Correction: The Greenhouse will always ensure correct ratio in all situations. Staff have received laminated cards with ratios as stated in DSS regulations. If the Director is counted among staff in ratio, interviews or meetings will be canceled or rescheduled.

Description: Based on interviews, the center failed to establish and implement a procedure to ensure appropriate supervision of all children in care.

Evidence:

1. Staff 2 stated on 10/13/20 three staff were on the playground supervising children. Staff 2 stated he/she was unsure how many children were on the playground at the time. Staff 2 did not have a list of names of children on the playground. At some point staff 1 went inside and left staff 2 and staff 3 alone with the children. When it was time to go in they did not do a head count. The two staff looked around the playground to see if they had all the children and then walked inside. About one minute after going inside a father of another child came in to let them know a one and a half year old child had been left on the playground.
2. Staff 3 stated on 10/13/20 three staff went outside with the children at 4:30 pm when the classrooms combined. When the other teachers left for the day staff 3 did not do a head count or get a class list before going outside. At 4:45 staff 1 went inside leaving staff 2 and staff 3 outside with the children. When it was time to line up and go in around 5:00 they looked around the playground but did not do a head count because they did not know how many children they had. They lined up and went inside. About one minute later a father of another child came in and said a child was left on the playground. Staff 2 later realized they had 16 children on the playground with the youngest one and a half and the child left alone on the playground was age one and a half.
3. Staff 1 stated there were 16 children on the playground on 10/13/20 and the youngest child was 21 months old. At the time of the incident they did not have a policy of keeping an attendance log with the classes.

Plan of Correction: Each class now has a laminated card with the class list. When a teacher leaves they physically hand the card to the staff taking over. Heads are counted at every move and transition. Children are alerted to the staff that are in supervision. Children?s names are marked off as they leave. Verbal communication as well as written documentation takes place between staff. The center now has an in place system to ensure communication between staff members leaving and staff coming in. This policy and physical list has created a system that works well.
