8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Comments:
An initial announced subsidy health and safety inspection (SHSI) was initiated on September 7, 2023 and concluded on October 2, 2023. On September 7, 2023 the licensing inspector was onsite from approximately 11:30am until 12:45pm. There were four children in care. One provider was present. One provider?s record was reviewed.

Violations:
Standard #: 8VAC20-790-180-D
Description: Based on observation and interview, the vendor did not maintain a written hard copy record of daily attendance that documented the arrival and departure of each child as it occurs.
Evidence:
Four nonresident children were present during the 09/07/2023 inspection. The vendor stated she did not have documented attendance.

Plan of Correction: I will start documenting the attendance of the children in care, including when they arrive and leave.

Standard #: 8VAC20-790-190-B
Description: Based on the review of four children?s records, the vendor did not ensure four children?s records contained the required information.

Evidence:
The records for Child #1 (first day of attendance 05/15/23), Child #2 (first day of attendance 03/06/23), Child #3 (first day of attendance 04/24/23), and Child #4 (first day of attendance 03/16/23) did not contain the address of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: I will ask the parents for the additional information to be added to the record.

Standard #: 8VAC20-790-420-A
Description: Based on documentation review and interview, the vendor did not have a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown, and shelter-in-place procedures.
Evidence:
The vendor stated she had not completed the emergency preparedness plan template. The vendor had printed the template. The emergency preparedness plan was blank.

Plan of Correction: I will complete the emergency preparedness plan and email it to the inspector.
