Comments:

This annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic. Preliminary data was obtained from the vendor on January 11, 2021. An annual subsidy inspection was initiated on January 13, 2021 at which time the vendor was contacted by telephone to initiate the inspection, and concluded on January 14, 2021. The inspector emailed the vendor a list of items required to complete the inspection. The vendor reported there were 32 children in care. The inspector reviewed three children's records and three staff records submitted by the vendor to ensure compliance with the health and safety regulations. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Standard #:	22VAC40-665-520-B-3
Description:	Based on record review, in three of three children's records reviewed, the vendor did not ensure that all required parent contact information was documented in each child's record. Evidence: The director confirmed that there was no documentation of the work addresses in the records for Child #1, Child #2 and Child #3.
Plan of Correction:	The parents will be contacted to get the employment addreses. Going forward we will make new registrtation packets and will be sure to include work addresses and work phone numbers for each parent.
Standard #:	22VAC40-665-580-H
Description:	Based on record review, in two of three staff records reviewed, the vendor did not ensure that complete the safety update course within one year of completing the Virginia Preservice Training. Evidence: 1. The director confirmed that Staff #2 completed the Preservice Training on 1/25/2019 and has not yet completed the safety update course. 2. The director confirmed that Staff #3 completed the Preservice Training on 1/3/2019 and did not complete the update until 3/14/2020.
Plan of Correction:	Staff #2 has already been contacted and we discussed that she will have the training completed by 1/19/2021. We will make sure that all staff complete the update within a year of the Preservice.
