Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on 1/26/2021 and concluded on 1/27/2021. A complaint was received by the department regarding allegations in the areas of supervision. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on staff record review, the center did not ensure that one of two staff records contained documentation of a central registry record check within 30 days of employment.
Evidence:
The record of staff #2 (start date 2/12/2018) did not have documentation of a central registry record check.

Plan of Correction: 1. Sent for a new Central Registry report for the staff person. 2. Sent copy of what was sent to [Licensing Inspector] - Email of Plan of Correction sent to [Licensing Inspector] 02/02/2021.

Description: Based on interviews, the center did not establish and implement procedures for appropriate supervision of all children in care, including daily intake and dismissal procedures to ensure safety of children.
Evidence: During the January 25, 2021 inspection, the following was determined: 1. The center representative and two staff confirmed on January 11, 2021 at approximately 5:30pm, Child #1 (three years of age) was found standing outside of the center alone by child #1?s parent. 2. The center representative reported on the evening of January 11, 2021, the front desk [staff #1] called a student and the teacher [staff #2] brought out the wrong student. The center representative also stated, the front desk [staff #1] was talking to a parent and did not see the child, the child walked out of the door using the push bar and [Child #1] was outside less than five minutes or maybe a minute. 3. During an interview on January 25, 2021 staff #2 reported the following: she has a two-way [radio] that they wear. On the evening of January 11, 2021, the person up front [staff #1] called a name and she [staff #2] asked to clarify the name. Staff #2 thought she heard child #1?s name. Staff #2 then got child #1 ready for dismissal and took child #1 to the first door of the classroom and directed the child to walk to the front desk. Staff #2 stated, ?I assumed she [staff #1] would have seen [child #1].? 4. During an interview on January 25, 2021 staff #1 reported the following: On the evening of January 11, 2021, she was helping a customer with a payment and heard someone banging really hard on the door and it was [child #1?s parent]. Child #1?s parent and the center representative went to the conference room. Staff #1 stated that she did not see child #1 walk out of the door and that child #1 was maybe outside for about a minute or two.

Plan of Correction: 1. The push bar is still in place. Added an additional lock that prevents the door from opening by the push bar. 2. Both teachers written up for the incident. 3. I created a new policy and procedure for dismissal. a. Call for student on 2-way. b. Teacher in preschool area repeat the name back to the front desk on 2-way. c. Prepare child to go home. d. Take child by the hand.
