Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A code compliance inspection was initiated on 02/11/2021 and concluded on 02/11/2021. The director was contacted by telephone to initiate the inspection. There were 13 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review, the center did not ensure three of three staff records had documentation of a completed sworn statement or affirmation prior to the first day of employment.
Evidence:
1. The record of staff #1 (start date: 8/12/2019) had documentation of a completed sworn statement or affirmation dated 10/3/2019.
2. The record of staff #2 (start date: 1/4/2021) had documentation of a completed sworn statement or affirmation dated 2/8/2021.
3. The record of staff #3 (start date: 1/11/2021) had documentation of a completed sworn statement or affirmation dated 2/8/2021.

Plan of Correction: Going forward, Saint Joseph's School's business manager will onboard JK and / or afterschool employees after receiving the results of their fingerprints and CPS. Additionally, the Sworn Statement of Affirmation for Child Day Programs will be completed at the time of onboarding.

Standard #: 63.2(17)-1716-B-3
Description: Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children.
Evidence:
During the inspection of three staff records and interview with the center representative, it was discovered there were no staff that received training in daily health screenings. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings.

Plan of Correction: Enrolled in the CHLD 2016: Health Screenings For Childcare Professionals through JTCC Community College Workforce Alliance. Attached you will find the paperwork documenting this action. St. Joseph School is scheduled to take this course [on] Friday, March 5, 2021. The Daily Health Screening Procedure will be developed upon completion of the Health Screening for Childcare Professionals course.
