Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A code compliance inspection was initiated on 03/10/2021 and concluded on 03/11/2021. The director was contacted by video call to initiate the inspection. There were 123 children present and 24 staff on 03/10/2021. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 6 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Description:	Based on staff record review, the center did not ensure six of six staff records had documentation of central registry results within 30 days of employment and / or a completed Sworn Statement or Affirmation prior to employment. Evidence: 1. The record of staff #1 (start date: 8/31/20) had documentation of central registry results dated 10/15/20 and did not have documentation of a completed Sworn Statement or Affirmation. 2. The record of staff #2 (start date: 8/31/20) did not have documentation of central registry results and had documentation of a completed Sworn Statement or Affirmation dated 11/2/20. 3. The record of staff #3 (start date: 11/1/20) did not have documentation of central registry results and a completed Sworn Statement or Affirmation. 4. The record of staff #4 (start date: 8/31/20) had documentation of central registry results dated 12/17/20 and documentation of a completed sworn statement or affirmation dated 11/4/20. 5. The record of staff #5 (start date: 11/1/20) did not have documentation of central registry results. 6. The record of staff #6 (start date: 1/11/20) did not have documentation of central registry results and a completed Sworn Statement or Affirmation.
Plan of Correction:	The Center understands going forward, all fingerprint based national criminal history search results and Sworn Statement of Affirmation forms should be complete and in good standing prior to anyone?s employment start date. All Central Registry background search results must also be received by the Center within 30 days of the employee?s start date and reflect the employee is in good standing. If an employee is a rehire at the Center within the past year, all noted forms are considered valid but invalid if the employee has been absent from the Center for more than one year?s time. An employee who is transferring from one Center to another is considered a new hire to the Center and all noted documentation must be in place appropriately.

The record of staff #4 (start date: 8/31/20) did not have documentation of fingerprint based national criminal history search results. 5. The record of staff #5 (start date: 11/1/20) did not have documentation of fingerprint based national criminal history search results. 6. The record of staff #6 (start date: 1/11/21) did not have documentation of fingerprint based national criminal history search results.
Plan of Correction:	The Center has responded appropriately to execute completion of stated documents- fingerprint based national criminal history search, Central Registry background search and Sworn Statement of Affirmation for described employees to comply as deemed necessary. The Center will have these documents in place within the 15-day period allowed.
